ABSTRACT

Organoids are three-dimensional (3D) biological structures constructed in vitro by stem cells, which partially mimic the function of real organs. Brain organoids are an extremely important branch of organoid research. This technology can differentiate pluripotent stem cells into the required cell types in a 3D culture environment, and self-assemble into structural bodies, but it is currently unable to fully replicate the structural and physiological features of the real human brain. The maturity of brain organoids may form consciousness, which poses ethical issues such as determining moral status and informed consent in brain organoids research. This paper elaborated on the research progress and future development direction of brain organoids, and proposed multidimensional governance strategies for ethical issues faced in brain organoids research from the perspectives of ethical principles, ensuring public informed consent, and legal supervision. By exploring the above issues, reference will be provided for formulating ethical principles to guide the research and application of brain organoids in the future.

ABSTRACT

Objective:To compare the clinical outcomes between replacement with a composite press-fit radial head prosthesis versus open reduction and plate-screw internal fixation in the treatment of adult Mason Ⅲ radial head fractures. Methods:The clinical data of 64 adult patients with Mason Ⅲ radial head fracture were retrospectively analyzed who had been admitted to Department of Orthopedics, Beijing Friendship Hospital from January 2012 to December 2019. They were 18 males and 46 females, aged from 32 to 58 years (average, 45.7 years). They were divided into 2 groups: 32 cases received mini-plate-screw internal fixation (internal fixation group) and 32 cases replacement with a composite press-fit radial head prosthesis (replacement group). At the last follow-up, elbow valgus angle, range of elbow motion, Mayo score of elbow function and visual analogue scale (VAS) pain score were recorded and compared to evaluate the postoperative clinical outcomes. Results:There was no statistically significant difference in the preoperative general data between the 2 groups, showing they were comparable ( P>0.05). All patients were followed up for 8 to 48 months (average, 18.7 months). The operation time was, respectively, (81.4±8.2) min and (68.9±7.3) min for the internal fixation group and the replacement group, showing a statistically significant difference ( P< 0.05). For the internal fixation group and the replacement group at the last follow-up, the flexion and extension angles were 95° to 125° and 100° to 140°, the rotation angles 135.3°±11.2° and 143.5°±12.8°, and the Mayo scores 79.2±3.8 and 83.4±3.9, all significantly favoring the replacement group ( P<0.05). The VAS pain scores before operation and at the last follow-up were 7.6±0.7 and 0.9±0.7 for the internal fixation group, and 7.9±0.8 and 0.7±0.6 for the replacement group, showing significant differences between preoperation and the last follow-up in both groups ( P<0.05). All the incisions healed by the first intention, with no postoperative infection. Internal fixation loosening with ulnar neuritis was reported in one case in the internal fixation group; peri-prosthesis absorption with no prosthesis loosening was observed in one case in the replacements group. Conclusion:In the treatment of adult Mason Ⅲ radial head fractures, although both replacements with a composite press-fit radial head prosthesis and open reduction and plate-screw internal fixation can lead to satisfactory results, the former may be more effective.

ABSTRACT

The epidemic of corona virus disease 2019 (COVID-19) is still ongoing, and infections among health care workers are not uncommon. How to complete the diagnosis and treatment of orthopaedic diseases and prevent the spread of the epidemic is a great challenge for orthopaedic surgeons. On the basis of exposure history and medical conditions of orthopaedic patients, the hierarchical control is very important for reduction of the exposure for the medical staff and patients. In order to ensure medical safety and reduce the consumption of materials for the epidemic prevention, we classified the hospitalized orthopaedic patients into three classes and six levels, to provide effective references for all levels of hospitals in orthopaedic clinical work.

ABSTRACT

Objective To quantitatively analyze the operation risks for femoral intertrochanteric frac-tures under the guidance of Acute Physiology and Chronic Health EvaluationⅡ( APACHEⅡ) . Methods A retrospective analysis was performed among the 226 patients with femoral intertrochanteric fracture who had re-ceived surgery between January 2013 and January 2016. They were 59 men and 167 women, aged from 69 to 106 years ( average, 75. 4 ± 6. 1 years ) . Their average APACHEⅡscore was 18. 3 ± 6. 3 ( from 5 to 34 ) . They were divided into 3 groups according to their APACHEⅡscores: 127 cases in the low risk group (≤ 15 ) , 68 cases in the medium risk group ( from 16 to 24 ) and 31 cases in the high risk group ( ≥25 ) . The mortality was compared between the 3 groups. The deaths and survivals were compared in each group in terms of age, oper-ation time, intraoperative blood loss, postoperative blood transfusion and combined internal diseases. Results Thirteen patients died during hospitalization, giving a mortality of 5. 7% ( 13/226 ) . The causes for death were heart attack in 6 cases, respiratory failure in 4, toxic shock in 2 and renal failure in one. The mortality in the high risk group ( 22. 6%, 7/31 ) was significantly higher than in the low risk group ( 1. 6%, 2/125 ) and in the medium risk group ( 5. 9%, 4/68 ) ( P <0. 05 ) . In the low risk group, the intraoperative blood loss was sta-tistically different between deaths and survivals ( P <0. 05 ); in the medium risk group, the intraoperative blood loss and postoperative blood transfusion volume were statistically different between deaths and survivals ( P <0. 05 ); in the high risk group, the age and intraoperative blood loss were statistically different between deaths and survivals ( P <0. 05 ) . Conclusions APACHEⅡcan be used to quantitatively evaluate the patients with femoral intertrochanteric fracture who usually suffer from intraoperative complications and concomitant in-ternal diseases. The most significant risk factor may be intraoperative blood loss.

ABSTRACT

<p><b>OBJECTIVE</b>To study the effect of oral fluid resuscitation with pyruvate sodium-glucose-electrolyte solution (PGES) on hemodynamics, organ functions and mortalities during shock stage in dogs with burn.</p><p><b>METHODS</b>In comparison of oral pyruvate sodium-glucose-electrolyte solution (PGES) with NaHCO₃-glucose-electrolyte solution (HGES), beagle dogs with intubation of the carotid artery, jugular vein and jejunum for 24 hours were subjected to a 50% total body surface area (TBSA) burn, and were divided into three groups: pure burn without fluid resuscitation (NR, n = 8), and two oral fluid resuscitation (each n = 10), in which dogs were given with Pry-GES (OP) or NaHCO₃-GES (OH) according to Parkland formula. The hemodynamic and organ functions were measured serially before burn and 2, 6, 8, 12 and 24 hours after burn at no anaesthesia state A. Twenty-four hours mortality rate following burn was also recorded.</p><p><b>RESULTS</b>Two hours after burn, the mean arterial pressure of NR, OH and OP group was (45 ± 8), (57 ± 8) and (80 ± 9) mmHg (1 mmHg = 0.133 kPa) respectively, which were significantly reduced (t = 16.967, 14.595 and 10.100, all P < 0.05) compared with those before injury ((42 ± 6), (144 ± 6) and (142 ± 6) mmHg respectively), the change of cardiac output, dp/dtmax of left ventricular contractility and intestinal mucosal blood flow had the same trend as the mean arterial pressure. The systemic vascular resistance and organ parameters (Cr, CK-MB, ALT and DAO) in all groups increased obviously (t = -46.894--2.465, all P < 0.05). All measurements of NR group kept worsening, and all died within 24 hours after burn; while those of two oral resuscitation groups had improved gradually (F = 0.001-1.600, all P < 0.05), OP group was significantly superior to OH group (F = 0.013-0.466, P < 0.05). At 24 hours after burn, 6 (6/10) survived in OP group, 4 (4/10) in OH group and 0 (0/8) in NR group.</p><p><b>CONCLUSION</b>The Pyr-GES may be superior to the standard NaHCO₃-GES in the improvement of hemodynamics and organ functions during oral resuscitation in dogs with 50%TBSA full thickness burn.</p>

ABSTRACT

Objective To evaluate the value of the physiological and operative severity score for the enumeration of mortality and morbidity (POSSUM) and P-POSSUM in predicting the risks of orthopedic surgeries for senile patients with femoral neck fracture. Methods A total of 108 patients with femoral neck fractures who underwent hip joint replacement were retrospectively studied using POSSUM and P-POSSUM scoring system to predict their mortality and complication rate. The difference between predictive value and observed value was analyzed by chi-square test.Meanwhile, the patients were divided into two groups based on their POSSUM scores. The differences between two groups were analyzed. Results According to POSSUM scores,47 patients were predicted to have complications(the mean rate was 43.52%),but only 37 did actually (the rate was 34.26%).There was no significant difference between predicted values and observed values (P =0.238).The predicted death toll was 11 cases (the mean rate was 10.19%),but actually only 2 patients died (the rate was 1.85%).Predicted value was higher than observed value. In terms of complications, death toll agreed well with the predicted values calculated by P-POSSUM (predicted death of 4 cases' the mean mortality being 3.70% ; actual death of 2 cases' the mortality was 1.85%) without significant difference (P =0.625).We divided the patients into two groups with the POSSUM scores 40,and there was no significant difference between predicted values and observed values (P =0.527,P =0.285).Conclusions POSSUM has better predictive ability of morbidity, but overestimates mortality.P-POSSUM more accurately predicts mortality than POSSUM.The predicted results of POSSUM and P-POSSUM scoring systems are satisfactory in the high risk group.

ABSTRACT

Objective To observe the effect and mechanism of HA1077 on bone mesenchymal stem cells (BMSCs) promoting wound healing. Methods (1) In vitro study: rats' BMSCs were har-vested and divided into three groups randomly, ie, control group, induction group and HA1077 group. In control group, culture medium was DMEM with 10% fetal bovine serum. In induction group, induction medium was DMEM which consisted of 10% FBS, 8μg/ml EGF and epithelium culture condition medi-um. In HA1077 group, culture condition medium was the induction medium pins 10 μmol/L HA1077. At day 7 after replanting, CK19 expression, PCNA expression and cell cycle among three groups were clarified by flow cytometry. (2) In vivo study: rabbit's BMSCs were isolated. Six pieces of full skin loss wound were created on the back of a rabbit, with three pieces of wound on the half side of back. The wound was round and 3 cm in diameter. Eight New Zealand rabbits were randomly divided into four groups, ie, control group (Group A), EGF control group (Group B), BMSCs group (Group C) and BMSCs + HA1077 group (Group D). Wounds were treated by normal saline in Group A and by EGF (50 U/cm2) in Group B. Besides EGF,1 ml of BMSCs suspension (2 × 106/ml) was used for wound re-pair in Group C. 0.5 ml HAIO77(20 μmol/L), BMSCs and EGF were used together for wound repair in GronpD. At days3, 7and 14 after injury, wound elosuroindex (WCI) was calculated. Results In vitro study:the positive rate of CK19 in the HA1077 group was (40.31±0.82)%, higher than (11.13±2.14)% in the induction group(P < 0.01). The PCNA positive rate and the S phase, cell quantity in HA1077 group were also higher than that in the induction group. In vivo study: WCI was (86.20±1.92) % in Group D at day 14 after injury, obviously higher than that in other three groups. Conclusions HA1077 can promote BMSCs' entering into S phase, expressing PCNA and differentiating and expressing CK19. In vivo combination of HA1077 with BMSCs may facilitate wound healing.

ABSTRACT

@#ObjectiveTo investigate atlantoepistrophic imaging features of patients with vertigo of cervical syndrome.Methods74 patients with vertigo of cervical syndrome were checked by photographs of cervical vertebrae of A-P position, lateral position and mouth open position, some patients were checked by CT three-dimensional reconstruction and/or MRI vertebral arteriography.ResultsImaging examination of these patients showed that physiolgical curvature of cervical vertebrae became straightened, width of both lateral masses of atlas was inequality, space of both lateral masses of atlas to odontoid process of vertebra dentata was also inequality, and inferior articular surface of lateral mass was not parallel with superior articular surface of axis. The CT could display the side displacement and anterior or posterior rotation of lateral masses of atlas. The cervical MRI vertebral arteriography could display the effect of side displacement and rotation of lateral masses of atlas on vertebral artery. If side displacement or rotation of vertebral artery occurred, the diameter of artery decreased.ConclusionThe diseased region of vertigo of the cervical syndrome is the atlas and axis, especially the side displacement and anterior or posterior rotation of the atlas.

ABSTRACT

OBJECTIVE To recombine J chain gene with HNP-1 into a new germicidal(molecule) J-HNP-1,which can connect with pIgR by J chain,so that by using pIgR as a "bridge" the J-HNP-1(germicidal) peptide can be transported into the epithelial cell of mucous membrane to kill the intracellular microorganisms, then the recombinant is inserted into the mammalian expression system.METHODS The J chain and HNP-1 cDNA were amplified from the plasmids respectively by PCR,then the two cDNA fragments were recombined into J-HNP-1 by recombinant PCR.The J-HNP-1 cDNA fragment was inserted into the(mammalian) expression vector pcDNA3.1(-)/Myc-HisC.RESULTS The J-HNP-1 recombinant was obtained by(connection) of J chain and HNP-1 cDNA by PCR.The recombinant J-HNP-1 cDNA was 786bp.CONCLUSIONS The recombinant J-HNP-1 cDNA and the construction of expression vector are the basis for the new bactericidal peptide production.
